Solihull CCG is delighted to have been informed that its entry into the HSJ Value in Healthcare Awards has been shortlisted! The Community MDT Project – ‘Collaboration in Care’ was submitted in the category ‘Managing long term conditions’ in partnership with Heart of England Foundation Trust, Diabetes UK and Public Health.
The team involved will now attend a judging day in March where they will present to a judging panel, along with nine other finalists in their category. They will find out if they have been successful at the prestigious Awards evening in Park Lane, London held in May.
Solihull has seen an increase in people with diabetes in the last three years of 11%, compared to the national average of 6%. Diabetes has been identified by Solihull CCG as an area where savings on current annual spend could be made.
Bernadette Faulkner, Commissioning Redesign Manager LTC at Solihull CCG and member of the finalist team said;
“The challenge was to redesign the diabetes pathway achieving financial efficiency while providing quality patient care.
Through collaboration between commissioners, primary and secondary care we have developed a high quality service which has enhanced the skills of the Primary Health Care Team and delivered improved outcomes and satisfaction for patients but without any appreciable increase in cost.”

“This has been an inspirational collaboration, showing the value of MDTs and it is wonderful that the results of the pilot demonstrate so many positive patient centred outcomes, but also financial and educational benefits.
This joined up and patient centric approach could be replicated across other workstreams and may also be a transferrable template for other practices to manage more effectively not only diabetes, but potentially other long-term conditions.”
